Mint a re-auth challenge for disabling HIPAA mode
Mints a five-minute, single-use re-auth challenge for the HIPAA-disable gate. Call this immediately before PUT /settings/hipaa with enabled: false, then replay the returned challenge_token in the X-Reauth-Challenge header — disabling HIPAA clears the covered-entity flag, the BAA link and the retention floor, so the server demands proof the operator is at the keyboard, not a stolen browser session. Enabling HIPAA and retention-only saves don’t need a challenge. Owner only.

Stripe-style idempotency token. Pass a stable, client-generated value (1-255 chars) to dedupe retries on transient timeouts. The same key+credential+path replays the original response for 24h on 2xx (5min on 4xx, 30s on 5xx). Returns 409 if a concurrent request with the same key is already in flight; replayed responses include the Idempotency-Replay: true response header.
