Mint a 2FA step-up challenge
Mints a short-lived (5-minute, single-use) step-up challenge after verifying a fresh credential proof — the request must carry the current password OR a current totp_code (at least one is required), not just session recency. Call this first, then pass the returned token in the X-Reauth-Challenge header when calling the 2FA disable or backup-codes regenerate operations gated on op (disable or regenerate_backup_codes).

Stripe-style idempotency token. Pass a stable, client-generated value (1-255 chars) to dedupe retries on transient timeouts. The same key+credential+path replays the original response for 24h on 2xx (5min on 4xx, 30s on 5xx). Returns 409 if a concurrent request with the same key is already in flight; replayed responses include the Idempotency-Replay: true response header.
