Create an outcome-scoring rubric for an agent — the success criteria the LLM judge evaluates conversations against (resolution, tone, scope adherence, escalation accuracy). Body: name, criteria_md (markdown), optional evaluator_prompt (omit to use the built-in judge prompt), optional scope (defaults to agent; use the org-scoped /outcomes/rubrics routes for inbox/voice/org). Max 20 rubrics per agent (422 RUBRIC_LIMIT over). 404 when the agent id is unknown to the tenant. Owner, admin, or developer role; audit-logged.

Stripe-style idempotency token. Pass a stable, client-generated value (1-255 chars) to dedupe retries on transient timeouts. The same key+credential+path replays the original response for 24h on 2xx (5min on 4xx, 30s on 5xx). Returns 409 if a concurrent request with the same key is already in flight; replayed responses include the Idempotency-Replay: true response header.
